Approval queue - notify user of action taken

I’ve just been curious about why this tick box is sometimes already checked and sometimes not? At first I assumed it doesn’t get ticked if the system assumes the registration is spam, and it auto ticks if the registration seems clean. But there are plenty of obviously clean registrations that aren’t automatically ticked.

So it isn’t a big deal but I’m curious?
